Sweet Home student athletes can get sports physicals on Wednesday, Aug. 6, at the high school gym.
Cost is $30. The physicals, good for two years, cover sports participation starting in the fall of 2014. School district policy requires that athletes must have a completed physical prior to participating in any athletics.
All funds collected for the physicals are being donated by Samaritan Health Systems to the Sweet Home Athletic Department.
Sweet Home Family Practice will not offer discounted sports physicals at the clinic and will not hold a special day for athletes to be seen for sports physicals. Athletes who cannot make the physicals offered on Aug. 6 will need to make an appointment at the clinic. Regular fees will be charged for any sports physicals performed at Sweet Home Family Practice.
On the day of the physicals, athletes must have the necessary forms filled out and signed by a parent or guardian. Checks should be made out to Sweet Home School District.
Athletes should plan to have their physicals performed during the times listed, based on the first letter of their last name: A through G – 1 to 1:30 p.m.; H through M – 1:30 to 2 p.m.; N through S – 2 to 2:30 p.m.; T through Z – 2:30 to 3 p.m.; All those unable to attend the above times – 3 to 3:30 p.m.
