Sweet Home paramedics transported a seriously injured Lebanon man to Lebanon Community Hospital Monday following a head-on collision about 1.5 miles east of Quartzville Road.
David B. Carpenter, 40, of Lebanon was extricated from his 1985 Nissan Sentra and taken to the hospital.
Richard L. Birky, 40, of Sweet Home was driving a loaded 1984 Kenworth log truck.
