The Sweet Home Fire and Ambulance District responded to calls for a motor vehicle collision in the 600 block of Main Street at 9:10 a.m, July 12.
According to the on scene battalion chief, Eli Harris, the van was pulling out of the Veterans of Foreign Wars and the Wells Fargo ATM area before colliding head on with a motorcycle that was heading westbound on Highway 20.
The driver of the motorcycle was taken to Lebanon Community Hospital with minor injuries. Both vehicles were taken from the scene with minor damages. The driver of the van, Janice Kay Novencido, 64, was cited for careless driving.
