SLCH Foundation accepting applications for scholarships

The Samaritan Lebanon Community Hospital Foundation and Auxilary are accepting applications for the following health career scholarships:

n John and Carol Dinges Medical Scholarship, for east Linn County students enrolled in an entry-level health care program at Linn-Benton Community College in such fields as certified medical assistant, certified nursing assistant, medical office specialist and dental assistant.

First preference will be given to seniors and graduates of Lebanon High School, but seniors and graduates at all high schools in east Linn County are invited to apply.

n Frank Girod, MD, Scholarship for east Linn County residents enrolled in medical school. The scholarship is named for the late Frank Girod, MD, a primary care physician who was instrumental in the establishment of Samaritan Lebanon Community Hospital.

Scholarship amounts have ranged from $1,500 to $8,000 per year. Previous recipients of the scholarship are now working across the country, from Hawaii to New York.

Applicants must submit an application, available from the foundation office at the hospital, as well as a written statement about their goals and any community service involvement.

n Rachel Easton, RN, Nursing Scholarship, for college students from east Linn County who are accepted into a nursing program. The $1,000 scholarship is named for Rachel Easton, who was a longtime nurse at the Samaritan Park Street Clinic in Lebanon.

All applicants must submit an official transcript including Fall 2011 grades, a one- or two-page typed statement discussing why you are pursuing a nursing degree and listing your goals; a completed application form and a Cost and Resource Estimate Work Sheet; any special circumstances you might like the Easton Scholarship Committee to consider; and a signed and dated letter of recommendation from someone who knows you well.

The deadline to apply for all of the above scholarships is April 30. Winners will be announced by June 1.

n Samaritan Lebanon Community Hospital Auxiliary is accepting applications for its scholarship program for students pursuing a health care-related career. The Deadline to apply is Thursday, March 1.

The auxiliary will grant five $1,000 student scholarships this spring. Those eligible to apply must have a permanent address in East Linn County and be enrolled in and actively attending classes in a program awarding a degree in a health care-related field.

Applications are available at the SLCH Volunteer Services office located at the hospital, 525 N. Santiam Hwy. Applicants must also submit the following, along with their application:

An official transcript including Fall Term 2011 grades.

A one- to two-page typed statement discussing why you are pursuing a degree in a health field and what your goals are.

A completed Cost and Resource Estimate Work Sheet.

Please explain any special circumstances, which will help the Scholarship Committee make a fair evaluation (optional).

A letter of recommendation signed and dated.
