Avamere, owner of Twin Oaks, is planning to close the is planning to close the Sweet Home care facility by October.
Twin Oaks, at 950 Nandina St., provides specialized skilled nursing services. Teams of geriatric therapists are state-certified in physical, occupational and speech therapy. In addition to post-acute therapy, Twin Oaks treats and cares for residents with Alzheimer’s and dementia-related diagnoses.
“The decision to close Avamere at Twin Oaks is due to year-over-year decline in census,” said Matt Hilty, president of Avamere Living. “We can combine our resources at our larger facility in Lebanon and people in the area also benefit from more post-acute care options through the expanded VA Samaritan campus.”
The closure will occur in 90 days. Notice was provided to the state two weeks ago.
“What’s most important during this transition is that all the patients and their families are aware of what is happening, that they know they have options, and that Avamere is here to help them during the transition,” Hilty said. “We’ve met with all the residents and their families. We’ve met with the staff, and we’ve met with the state to ensure everyone is on board with this difficult decision; and we have overwhelming support from all parties involved.
“We have jobs for everyone who wants to continue to work with Avamere, and we are assisting with placement for residents at our care facility in Lebanon or with other care providers in the area depending on the choice of each resident.”
